Abstract

Circular shunt in Ebstein anomaly is associated with high in-utero mortality. Transplacental indomethacin has been reported to improve perinatal outcomes by inducing ductus arteriosus constriction. We report two fetuses with Ebstein anomaly and circular shunt treated with antenatal oral indomethacin: one diagnosed at 33 weeks' gestation with early hydrops and another at 29 weeks' gestation with advanced hydrops. The markedly different outcomes illustrate the potential benefit of prenatal indomethacin when initiated early in the disease course and emphasize the importance of early fetal cardiology referral and proactive management in this high-risk population.
