Abstract

Biofilm formation on implant devices and antibiotic resistance of bacteria make managing implant-related infection challenging. Irrigation of the infected joint with antiseptic agents is crucial in periprosthetic joint infection (PJI) with debridement, antibiotics, and implant retention. Various antiseptic agents are commonly used for irrigation, but the optimal choice, concentration, and irrigation time for complete biofilm elimination still need to be determined. The current research aims to investigate the effectiveness of povidone- iodine (PVP) irrigation on stainless steel implants coated with Methicillin-resistant Staphylococcus aureus (MRSA) using an in vitro method. The mature biofilms were analyzed using a scanning electron microscope (SEM) to study their morphology and physiology. Later, the mature MRSA biofilm was treated with PVP irrigation at three different time intervals (1, 3, and 5 minutes) with six different doses (0.3%, 0.5%, 0.75%, 1%, 2.5% and 5%) to compare its efficacy using standard treatment methods. The results showed that low concentrations of PVP (0.75%) treated for a duration of 5 mins, effectively eliminated the MRSA biofilm. It suggests that irrigating with low-concentration PVP could be a beneficial treatment for PJI in the presence of biofilm. Additionally, the study provides evidence that irrigation with PVP is critical in removing biofilm during PJI treatment.
